Clinical Observation of Chronic Myelocytic Leukemia in Children


Abstract
The following results were obtained on clinical assessment to 25 cases of chronic myelocytic leukemia which had been admitted to the Dept. of Pediatrics, SNUH, from Jan., 1955 to Oct., 1981.
1. During this period, 476 leukemic patients were admitted, so 5.3% of patients were CML.
In the 25 cases of CML, 13 cases were the juvenile type and 12 cases were the adult type.
2. Male to female ratio was 13 to 12 with no sex difference. But in the juvenile type, 9 patients were male and 4 patients were female. In the adult type, 4 patients were male and 8 patients were female.
3. Chief complaints on admission were abdominal distention and mass (72%), fever (12%0), coughing (12%6), dyspnea (12%) , pallor%), epistaxis (4%) , purpura (4%) , and bloody stool (4%) in order of frequency.
4. The following differences were observed between the juvenile and the adult type.
a) Age: In the JCML the median age was 1 year and 7 months (ranging from 2m to 3y 8m), and in the ACML the median age was 9 year and 9 months(ranging from 2y 11m to 13y).
b) Leukocyte count in peripheral blood: In the JCML the median leukocyte count was 54, 700/mm3 (range : 19,700-103,200), and in the ACML the median leukocyte count was 224, 600/mm3 (range : 2, 200-528, 600).
c) Platelet count: In the JCML the median platelet count was 78, 000/mm3 (range : 30, 000¡þ278, 000), and in the ACML the median platelet count was 400, 000/mm3 (range : 52, 000 - 1,366,000).
d) Marrow myeloid-to-erythroid ratio: In the JCML the M: E ratio was 2 : 1-10: 1, . and in the ACML the M:E ratio was 5.8: 1-57.6: 1.
e) Splenomegaly: In the JCML the median size of spleen was 8.7cm, and in the ACML the median size of spleen was 22.8cm.
f) Monocyte in peripheral blood: In the JCML the monocyte count was 8-32%, and in the ACML the monocyte count was 0-6%.
g) Basophil in peripheral blood: In the JCML the basophil count was 1-2%, and in the ACML the basophil count was 3-32%.
